The exact timing of the use of medication was recorded using the smart inhaler by Adherium (currently available as “Hailie”; Multimedia Appendix 1). This is an inhaler add-on that records when the inhaler is used. The patient kept using their regular inhaler (including canister) prescribed by their asthma health care provider. The smart inhaler itself is a small extra “casing” for the regular inhalers.

An aromatherapy inhaler is a portable, plastic, lipstick-like container that houses a cotton wick soaked in essential oils. The inhaler enables users to smell the aromas without getting their hands dirty or spilling the oils. Similar “aromasticks” have been used in British hospitals to control symptoms of nausea, insomnia, and anxiety, and to induce relaxation [31,32].

Sanchis et al [10] conducted a systematic review in 2016 to assess the most common errors in inhaler use among patients with asthma and those with COPD treated with MDIs. They concluded that incorrect inhaler use is unacceptably high outside clinical trials and does not seem to have improved over the past 40 years [10].

A Study to Investigate the Prevalence of Device-Specific Errors in Inhaler Technique in Adults With Airway Disease (The SCORES Study): Protocol for a Single Visit Prevalence Study

They will be accompanied by a member of the study research team, and the same person will both assess and correct any identified errors in the inhaler technique. The study team members will also record all relevant study data into the CRF. The participants will be asked to demonstrate their inhaler technique using a single-use placebo inhaler device. This is standard clinical practice, and placebo devices, which are identical to the inhaler, are available for all inhaler device types.

Effectiveness of Text Message Reminders on Adherence to Inhaled Therapy in Patients With Asthma: Prospective Multicenter Randomized Clinical Trial

All patients received a Smart Track device (Smartinhaler [Adherium Ltd]; Figure 1) that clipped on their ICS/LABA inhaler. Smart Track is an electronic device that allows measuring adherence to the inhalers. Each puff is recorded in the device memory. Smart Track records the date and time the inhaler was used and the number of puffs taken. Smart Track features include reminders, onscreen questions about asthma control, and medication feedback viewing online, but these have been deactivated.
