A total of 6 (29%) of the 14 external validations assessed discrimination, with all using the c-statistic. Birks et al [21] externally validated Colon Flag at multiple time intervals between the most recent blood test and diagnosis in a UK sample, reporting c-statistic=0.844 at 3‐6 months, which reduced to 0.751 at 23‐36 months [21]. Kinar et al [27] also externally validated the Colon Flag using UK data and reported a similar c-statistic (0.81) at 3‐6 months before colorectal cancer diagnosis [27].

Biomarkers will include immune functioning indicators (IL-1β, -6, and -8, tumor necrosis factor-alpha, C-reactive protein, serum amyloid A, vascular cell adhesion molecule 1, and intercellular cell adhesion molecule 1), metabolic functioning indicators (glycosylated hemoglobin, high-density lipoprotein, and low-density lipoprotein cholesterol, and triglycerides), adiposity (BMI, body fat percent, and waist-to-hip ratio), and blood pressure.
