Predicting Social Determinants of Health in Patient Navigation: Case Study

If we call the action of observing a patient and their associated data “x,” then a patient navigator will try to take “x” and predict a set of SDo Hs “s.” Machine learning algorithms try to replicate this human prediction process by finding a function that statistically fits a large number of human predictions so that when given a particular “x” it can determine “s” as accurately as possible (f(x)=s).
