The search was restricted to studies published in English as restricting it to English-language publications had little impact on the effect estimates and conclusions of this systematic review [25]. The study design was not restricted in the search strategy. The full search strategies are available in Multimedia Appendix 2. To ensure that all possible trials were considered for inclusion, citation tracking was conducted, and the reference lists of all the included trials were also checked.

All questionnaires were offered in English and Spanish languages.
The initial questionnaire (developed for the Delaware and Maryland baseline survey) consisted of items drawn from existing sources, such as the American Community Survey, the Behavioral Risk Factor Surveillance System, the NSFG, the PRAMS, and the Delaware Household Survey. A copy of the questionnaire can be found in Multimedia Appendix 1.
