Using the EMRs, we identified 468,162 individuals who carried at least 1 diabetes-related diagnosis based on the International Classification of Diseases, Ninth and Tenth Revisions (ICD-9 and ICD-10, respectively) between April 1, 2012, and December 31, 2016.

The study cohort included patients meeting the following criteria: (1) admitted to a KPNC hospital between June 15, 2018, and April 29, 2020, (2) at least 18 years of age, (c) diagnosis of incident ischemic stroke (ICD-10 codes: I63.xx, see Multimedia Appendix 1), and (4) for patients who experienced interhospital transport, the first hospital stay was at a KPNC hospital. Patients with in-hospital deaths were excluded from the final analysis of the study cohort.

This study was funded in June 2021, and recruitment began in April 2023. As of November 2024, we enrolled 90 participants. Data collection to assess the intervention’s effectiveness is expected to end in June 2026. Implementation evaluation is expected to conclude in December 2026.
