Vitality UK is a health and life insurance company that offers Vitality Active Rewards, a reward program for staying active. Vitality’s Active Rewards program was the subject of an observational real-world study in 2019. The researchers found that larger long-term incentives alone did not increase physical activity or prolong engagement as effectively as when combined with smaller immediate incentives.

Rewards are reinforcers of behavior that can be given for attempts to reach a goal and for reaching the goal [9]. In line with the CALO-RE taxonomy [9], only rewards that incentivize performing PA behaviors were considered and not rewards that incentivize study participation. A total of 12 studies (29 different intervention arms) met these criteria. Regarding technical implementations, 6 different delivery systems were used, such as websites (n=14).

This can be aligned to the long- and short-term rewards leading to motivation levels: those focusing on longer-term rewards (fitness and appearance) are less likely to continue with a physical activity program than those focusing on the instant rewards of enjoyment and social interaction. This can be explained by systematic biases toward immediate reward in human behavior, known as present bias [8].
