Evaluation of a Web-Based Medication Reconciliation Application Within a Primary Care Setting: Cluster-Randomized Controlled Trial

To address the limitations of existing tools and improve medication reconciliation within our primary care environment, we developed an EHR-integrated web-based reconciliation application, Med True (MT), to facilitate the process of reconciling EHR medication lists with other sources, including patient-reported medications within a primary care environment. We designed this tool to have both staff- and patient-facing interfaces.
