To facilitate the relationships between hospital and community pharmacists, practical information sheets are shared, including a contact sheet, checklist for dispensing emicizumab, and liaison sheet between the community pharmacist and the care center. The hospital pharmacy is informed by the care center about the transition of dispensing emicizumab in community pharmacy.
