The ASPIRE study offered three programs: (1) self-directed weight loss (SDWL), (2) app-supported weight loss (ASWL), and (3) coach-supported weight loss (CSWL). Participants self-selected a program. Eligibility and interest were determined via self-report on surveys and screening phone calls. Those who opted for weight tracking were mailed a scale with the cellular capability to transmit their weights. The primary outcome is the 6-month weight change in the CSWL program.

A short web-based interview with the program’s health coach was offered to all participants during the first 2-3 weeks of the study to establish coach connection and accountability and to provide participants with an opportunity to ask questions. During the interview, the primary goals, main concepts, and the program’s approach to diet and weight management were explained. In addition, participants’ strengths and potential barriers to participation were discussed.

The health coach also provided informed consent to be interviewed. All data were electronic, deidentified, and stored on the institutional servers of the corresponding authors. There was no compensation for participating in this study. Generative artificial intelligence was not used in any part of the manuscript writing.
A total of 16 participants were recruited for the study.

Mindfulness Coach, an i OS- and Android-based mobile app, is designed to deliver a mobile mindfulness training course designed by researchers from the US Department of Veterans Affairs [21,28]. The senior author of this paper, ER, was part of the team that developed the most recent version of the app. Initially developed among other mobile apps in collaboration with the US Department of Defense [28], Mindfulness Coach is freely available to the general public for both i OS and Android operating systems.

All adolescents (ages 13-17 years) who met the following inclusion criteria were eligible for inclusion in the study (N=689): (1) started mental health care (first synchronous event) with Bend Health Inc between January 1, 2023, and October 1, 2023 (9 months); (2) had at least 1 synchronous session with a Bend Health Inc coach or therapist during the study time frame; and (3) completed the assessment of eating behaviors before the start of care (baseline).

Each responding member’s duration in care was assessed as the number of months between their first synchronous session with a Bend Health care provider (eg, BCM, coach, or therapist) and their survey completion date. Descriptive statistics of care with Bend Health, Inc are reported for 2022. Survey respondents’ demographic information and patterns of participation were then compared to those who were sent the survey but did not respond using paired t tests and chi-square tests.

To determine fidelity, each coach will video-record their delivery of an ACHIEVE-D group session once a month. Two research team members will view each recording and use the ACHIEVE-D Curriculum Fidelity rating tool to assess performance on key elements.

After downloading the mobile app, members can start texting with a behavioral health coach within minutes of requesting to connect. Ginger coaches are full-time employees who have an advanced degree in a field related to mental health or have accredited coach certification. While many members are solely engaged with text-based coaching services, some will request or require escalation to clinical services (teletherapy or telepsychiatry) depending on preference or clinical severity.

All the interventions used were reviewed by 2 authors (JA and HI), who then grouped them into the following categories: smartphone apps, trackers (weighing scale, step tracker, or bite counter), behavioral therapy or advice (podcasts, telephone calls, booklets, SMS text messages, and in-person meetings), feedback (SMS text messages, email, oral, or written by a coach), self-monitoring, social support (social media or web-based forum), meal replacement, and financial incentives.
