Bipolar disorder (BD) is defined as a chronic mental illness with recurrent mood episodes, with manic, hypomanic, and depressive episodes alternating with euthymic periods. The illness mostly begins in adolescence and young adulthood [1]. It is estimated that BD I and II disorders occur in 2% of the world’s population, and another estimated 2% has a subthreshold BD [2]. In the Netherlands, the prevalence of BD in the adult population is 1.3% [3].

Using Machine Learning Imputed Outcomes to Assess Drug-Dependent Risk of Self-Harm in Patients with Bipolar Disorder: A Comparative Effectiveness Study

A recent systematic review showed that bipolar disorder (BD) may be associated with the highest suicide risk among all psychiatric disorders, with over 15%-20% of deaths attributed to suicide and the standardized suicide rate being 20 to 30-fold greater than in the general population (0.2-0.4 per 100 person-years) [4]. Another review found that up to 20% of individuals with BD end their life by suicide and 20%-60% attempt suicide at least once in their lifetime [5].

In May 2017, r/depression has been active for 8 years with 178,921 subscribers [63], r/bipolar has 24,724 subscribers and was formed 8 years ago [64], and r/schizophrenia has 7036 subscribers and has been active for 7 years [65]. Thus, we selected r/depression, r/bipolar, and r/schizophrenia as the main communities of interest for investigating the written communication challenges faced by individuals in online mental health communities.
