HIV and viral hepatitis remain large public health challenges in the World Health Organization (WHO) European region. An estimated 3 million people are living with HIV, 10.6 million with hepatitis B, and 8.6 million people with hepatitis C [1-3]. In the European region, the public health burden of HIV and hepatitis B and C is particularly high in the Eastern European and Central Asian countries [4]. HIV and viral hepatitis share similar modes of transmission and coinfections are common [5].

The aging global population also includes a growing number of older adults living with HIV [2]. Due to advancements in combination antiretroviral therapy, the life expectancy of people living with HIV has significantly increased [3], leading to the first generation of people living with HIV now reaching geriatric age [4]. This demographic shift brings unique challenges as older adults living with HIV experience a complex interplay of age-related comorbidities and HIV-related health issues [5-7].

HIV/AIDS has claimed 42.3 million lives since the beginning of the epidemic and 630,000 lives in 2023 alone [1]. In addition, the continuous transmission and heightened rates of new HIV infections across different countries remain a significant concern [1,2].

With the availability of effective antiretroviral treatment, mortality among people with HIV has decreased considerably; in Japan, the 10-year survival rate for people with HIV no longer differs significantly from that for the general population [1]. Further, modern antiretroviral therapy is able to suppress viremia sufficiently to prevent sexual transmission of HIV [2]. Yet, public perception may not necessarily reflect such progress.

Since the epidemic began, more than 40 million lives have been lost due to HIV-related illnesses [2]. Lifelong antiretroviral treatments require long-term health care services and medications [3].

HIV remains an important health concern and an area of public health efforts in China, with a particular focus on HIV transmission among gay, bisexual, and other men who have sex with men due to high and sustained risk of HIV infection. A cross-sectional survey of 47,000 men who have sex with men from 61 cities in China in 2008 to 2009 found an overall HIV prevalence of 4.9% [1].

Epidemic modeling studies [2-4], our National HIV/AIDS Prevention Strategy [5], and the Centers for Disease Control and Prevention (CDC) Ending the HIV Epidemic initiative [6] all emphasize the critical role of pre-exposure prophylaxis (Pr EP) in the national HIV prevention response.
