Counselors conducting E-TAU treatment are asked to provide counseling as usual for a period of 4 to 6 weeks but are asked not to use CAMS (eg, suicide status forms and sitting next to the client) or CC-DBT (eg, diary card and DBT skills)–specific strategies. Counselors are informed that all their sessions are recorded and some sessions will be randomly rated on adherence to ensure that no CAMS and CC-DBT strategies are used.

The cultural adaptation of the DBT skills was based on the EVF, including the following dimensions: language, persons, metaphors, content, concept, goals, method, and context [34]. For example, the language translation of certain DBT terms emphasized conveying the meaning accurately rather than word-to-word translation.

The content of the app was informed by theoretical approaches that appeared promising in treating self-harm—in particular, cognitive behavior therapy (CBT) and dialectical behavior therapy (DBT). The structured nature of these therapies facilitated the incorporation of core therapeutic techniques into a mobile app. Blue Ice therefore includes ideas from CBT (behavioral activation, thought challenging, and mood lifting activities) and DBT (mindfulness and distress tolerance).
