Implementing the H&P 360 in Three Medical Institutions: Usability Study
To significantly improve the health of all, it is critical to emphasize addressing the broader SDOH inequities—those created and sustained by structural racism and the marginalization of specific groups, including women, Black, Hispanic/Latino, LGBTQ+ (lesbian, gay, bisexual, transgender, queer or questioning, and other diverse sexual orientations and gender identities) individuals, people living with disabilities, and other populations [4,5,10-12].
