In medical and psychological research, generating valid study results by collecting data from large representative samples is essential [13]. For many research teams, recruiting the number of participants needed on time and within budget limitations becomes a major challenge [14]. Over 80% of trials worldwide fail to complete patient recruitment within the initially planned time frame, with insufficient case numbers being the primary reason for discontinuing 55% of all studies [15].

Patients in the intervention group filled in the Physical Activity Readiness Questionnaire (PAR-Q) before starting the intervention to determine whether they could safely perform physical activity [16,17]. Patients who were scheduled to receive instructions on increasing physical activity and muscle strengthening through the app were contacted by a member of the research team when answering yes to one or more questions in the PAR-Q.

Missing data analysis was conducted, and variables with more than 30% missing values were excluded to enhance model robustness. Missing data were imputed using Multivariate Imputation by Chained Equations. See Multimedia Appendix 1 for details on the percentage of missing data for each variable before imputation.

Because of the different modes of administration of these treatments, each group receives both an injection and oral tablets. The study therefore comprises three arms: (1) a “toxin” group treated with an injection of botulinum toxin and placebo oral baclofen; (2) a “baclofen” group treated with oral baclofen and an injection of placebo botulinum toxin; and (3) a double “placebo” group treated with placebo oral baclofen and an injection of placebo botulinum toxin.

A collaborative initiative between the HCFMUSP and the UK Government’s Better Health Program developed a Digital PHC proof of concept. This initiative focused on implementing telemedicine service in Santarem, Pará, Brazil, due to the lack of health care services in remote areas.
